To begin the Online Application process, families will need to create an account using the link below. Create A New Student Application Account for your child. You will then have the flexibility to log in and out of your account, access your open application. After submitting the application, you will be able to track online your admissions status at the school by logging in to your account. A non-refundable fee of $50 must be submitted with each application.

• Student's most recent report card
• Most recent standardized test scores (grades 2-8)
• Birth certificate
• Copy of 504 Plan or IEP (if applicable)
• Psychological Evaluation (if applicable)
• Copy of Legal Guardianship/Custody Agreement (if applicable)
• State required health records form #2040/680
Paperwork and all forms can be emailed to bthornhill@harmonprep.org during the application process. If you are unable to upload, all paperwork and forms should be submitted at the main office. Please be advised that class sizes are limited and waiting lists are likely. Applying early is strongly encouraged. Openings, if they occur, will be filled from the waiting list.
New applications are reviewed once all forms, documents and fees have been submitted. Once the application has been reviewed families can move forward to the Academic Screening Process.
